This is a video about Enoxaparin (Lovenox), an anticoagulant, low molecular weight heparin. A student nurse is instructed a patient on this medication. This is a college project for Great Basin College for Pharmacology NURS 155.
Hi michelle my name is pam i’m one of the student nurses at great facing college how are you doing today i’m all right a little better now okay very good so um here is a handout um about the medication that you’re gonna be going home with um i’m just gonna go over the highlights and then you can go home and read it more in depth and if you have any questions
You can call me back at any time okay and i’ll be able to answer more questions so the provider your provider has ordered you in oxiparin which is the generic name for levinox so i’m gonna say love an ox from here on out because the other one is about five syllables long and i would rather say love and ox it’s easier to remember okay okay explains to me so
Lemon ox is a low molecular weight heparin and heparin is an anticoagulant and that what that means is that anti-coagulant meaning that you don’t want something to clot right or clump together okay um so that’s what this medication is going to do we do not want your blood to clot in your veins and potentially cause serious injury to your body so what this
Magnification is going to do is going to um prevent the clot from forming yeah okay so we don’t want that to form so what’s going to happen is the reason you’re going to be taking this is because we don’t want you to get a blood clot that sounds great i would like to prevent that and not do that again either yes so um there’s reasons why you would we want you
To be put this on a prevention which we call prophylaxis is um one if you already had a blood clot we would put this on you another one is if you had irregular heartbeat okay um or a mechanical heart valve okay um so those or you can you know that could be a bridge also if the doctor wants you on coumadin to keep your blood from clotting faster um so it could
Be a bridge from coumadin which is a the brand name for warfarin okay okay so because warfarin is a pill that you take it’s gonna take longer to build up into your system to get a therapeutic level okay so in the meantime we’re going to give you lemon ox which is an injection under the skin subcutaneously into the fat tissue you’ll be giving that to yourself
And we’ll be teaching you briefly how to do that um so that’s really quick acting it’s going to help prevent the clotting now until the coumadin catches up in three to five days to take over and then you’ll stop the injections and just continue on with your pills of the coumadin okay okay all right so a blood clot mainly is is when platelets and and fibrins
They start to get thick and start clotting together and clumping and then it can block one of your your veins and your legs um and then if that breaks off into your blood system it could go up into your lungs and potentially be very harmful to yourself yeah we don’t want that to happen no no no no so um what we’re hoping for as is for the therapeutic reasons
Why you’re gonna get this infection is just so you don’t get this blood clot okay so for the from the history that we received from you okay you’re at a high risk so um you are at a high risk for possibly getting a blood clot so right now we just want to have prevention yeah okay yeah that sounds like a plan so how do you take this so like i said it’s gonna
Be by a pre-filled syringe so don’t thinking right you don’t have to draw anything up it will be coming in a package you’re just going to open up the package the directions will be on the box and inside the insert basically what you’re going to do is you’re going to clean off an area about two to three inches away from your belly button you can rotate left side
Right side up and down on your belly area so what we like to call is like your flank or the sides of your belly and yes so we’re looking like in this area here and over here okay and you would just clean that area and let the alcohol dry okay and you would just take the cap and pull it straight off okay and at a 90 degree angle you would just inject and hold it
There and then push the plungers um until you can’t push anymore okay and then you’ll take that out okay and you don’t want to aspirate meaning you don’t want to pull back on the plunger while the needle’s in right and you don’t want to massage the area after you pull the needle out okay okay you just want to leave it alone okay now there is potential for some
Slight bruising because it does um prevent your blood from clotting so you might have some bruising on your belly if you know you were to take it for a longer period of time okay um if you have any questions we are going to be having a home health nurse come to your house just to if you have any other questions you can ask her to demonstrate it for you okay
And so the doctor um the way we determine how much you’re gonna get is off of your weight okay so the doctor will figure that out for you so you don’t have to worry about that okay oh my gosh i think they’ve got a child oh my god it’s been six minutes okay so side effects of this uh levinox is bleeding so makes sense we don’t want your blood to get um clumpy
Okay so if it doesn’t clot then you have the higher chance of it bleeding of bleeding longer so when you go to shave you don’t want to use a razor you want to use a shaver you don’t want to do any type of hard physical activity that could cause you to to bleed or get a cut or injury and some of the biggest side effects are bleeding so that’s all i’m going to
Tell you now if you accidentally take an overdose or take it too many times in a row and you can’t stop bleeding then you need to call 9-1-1 and give us a call so that we can give you the antidote which would be a pro proto-mine right which you don’t need to remember that just know that if you’re bleeding you need to get to us immediately okay uh we will be
Having your blood checked okay to see how thick or thin the blood is and to check to see um if the medication that you’re taking is effective um there is one more thing that you need to know that if you are taking any type of other medications on your list that we need to know about would be like your aspirin naproxen any type of pill that can cause further
Bleeding and also if you’re taking um herbs um which would be like your ginseng ginger garlic um in ginkgo biloba yeah no garlic and i are friends that’s just good um so you’re not allergic to any pork products are you allergic to any other medications no no okay very good um all right then so do you have any questions no it sounds like you know i’m not sure
How to do this but you know home health nurse i guess we’ll hold my hand and walk me through it okay and um we can um i can come back later and i can when i go to give you your injection while you’re here in the hospital i’ll let you observe what how i’m doing it okay and then um we can even let you uh do it yourself right before you leave the hospital okay
