In this episode, I discuss a medication known as diltiazem (Tiazac).
Hey everybody welcome back to drug talk as always i’m your host garrett campbell today we’re gonna be discussing the medication known as diltiazem its brand name is tyzack now before i talk about the medication itself just keep in mind that this channel is for information purposes only and not to be used as a source for recommendations for your personal healthcare
So diltiazem is a slow-acting calcium channel blocker it blocks calcium ion in flux during depolarization of cardiac and vascular smooth muscle it also decreases peripheral resistance and causes relaxation of vascular smooth muscle in terms of indications for use diltiazem is indicated to be used in the treatment of hypertension or high blood pressure it can
Also be used in chronic stable angina or chest pain this medication can be used for atrial arrhythmias and finally it’s also indicated to be used for supraventricular tachycardia now before somebody was to use diltiazem there are some contraindications they must clear as well as some precautions and warnings that they should be made aware of medication cannot be
Used during an acute myocardial infarction or heart attack or if a patient has pulmonary congestion on an x-ray this medication cannot be given intravenously within a few hours of also giving intravenous beta blockers bowtie as m cannot be given to patients experiencing cardiogenic shock and it also can’t be given to patients who have second or third degree heart
Block unless they have a pacemaker patients with a hypersensitivity to diltiazem or any other component of the product would not be able to use this medication and it also can’t be given to patients who have symptomatic hypotension so typically this would be a systolic blood pressure less than 90 now in terms of precautions this medication is on the biers criteria
Which is a list of medication that the elderly population should either avoid or use cautiously we would want to avoid giving this medication for heart failure in the elderly population as fluid retention may occur and exacerbation of heart failure may also occur it should be noted that super event trick euler arrhythmias may occur during iv administration of
This medication we would want to avoid giving diltiazem intravenously with other intravenous medications that are also known to reduce peripheral resistance we would typically want to avoid giving this medication with beta blockers as they would have an additive effect on the heart rate dermatological side effects such as exfoliated dermatitis have been reported
With the use of delta ism we would want to use this medication cautiously in patients who have hepatic or liver impairment due to the increased risk of toxicity we should also use this medication cautiously in patients who have renal or kidney impairment now once somebody is cleared of the contraindications and made aware of the precautions and warnings and they
Start to use diltiazem it is available in several dosage forms but typically they would receive their dose orally if somebody is using the extended-release capsule for hypertension or high blood pressure they would typically start off with the dose of 120 to 240 milligrams once daily usual maintenance dose would be somewhere between 120 and 540 milligrams daily
If somebody is using the extended-release capsules for chronic stable angina they would typically use 120 to 180 milligrams once daily initially dose can be titrated every 7 to 14 days up to a dose of 540 milligrams once daily so as with all medications there are some side effects or adverse reactions that patients may experience while using diltiazem so i’ll go
Over some of those here now rady arrhythmias fall between 1 and 4 percent 4 to 8 percent of patients may experience peripheral edema dizziness falls between 3 & 6 percent and up to 5 percent of patients may experience a headache 2 percent may experience a cough and 5 percent may experience fatigue of some more serious but rare side effects would be congestive
